Physical Therapy Evaluation
60-90 minute appointment for evaluation and assessment of your condition, including your initial treatment
$200
Follow Up Visits
Standard visit time is $150 for 1 hour appointment
Times and pricing can be prorated or adjusted as needed, with minimum appointment charge of $60
Additional Fee for visits >30 miles from Goodyear, AZ may apply
VIP Treatment Packages
Customized in Home Physical Therapy treatment plan packages customized for those that need full service convenience, priority scheduling, and maximized recovery rate to restore optimal function on your schedule. Includes manual therapy treatments, custom exercise prescription, and personalized "on call " availability.
Package price to be determined upon evaluation, as every condition is individualized.
